If you have an urgent medical problem, are a new patient or would like to book an appointment for a specialist service, please contact the surgery directly on 071 9168220.

How to request an appointment online

Arranging your appointment online is the most convenient way to request a routine appointment with a doctor or nurse.

Complete and submit the form to request your appointment.
When an appointment or call has been booked for you, an email will be sent with the details. You must ensure that you do not have a spam-blocker installed or any other kind of software that will reject this automatic email.
Please note that video/telephone consultations must be paid for in advance. Payment instructions will be emailed to you with your appointment confirmation. (Does not apply to patients who have a medical or doctor visit card).
If you are unable to keep the appointment please telephone the surgery and cancel as soon as possible.
